To qualify for the Head and Spinal Cord Injury (HASCI) Waiver you must be determined eligible for services through the SCDDSN Head and Spinal Cord Injury Division. You must request HASCI Waiver services through your Service Coordinator and be eligible for Medicaid or already covered by Medicaid. You must meet Nursing Facility or ICF/MR level of care and have urgent circumstances that affect your health or functional status, which could be life-threatening. You must be dependent on others to provide or assist with critical health needs, basic activities of daily living or require daily monitoring or supervision to avoid being placed in an institution. You must need services not available from or adequate with existing resources, including family, private means, and other agencies/programs to meet you basic needs that allow you to remain in the home. You must have adequate natural supports to live safely in a private home or other community setting with the type/amount of services available form the HASCI Waiver.
